On 2023-11-04,I sustained an accident at the construction site,resulting in a serious injury.After being rushed to the hospital,I was diagnosed with a broken leg and required several weeks of rest and treatment.

During my recovery period,I had to inform my supervisor and HR department about my injury and provide them with a written notice of my leave.As per our company's policies,we promptly informed our HR department and Workplace Safety & Health Department about the incident and requested for an extension of my leave period.

After receiving medical treatment and advice from our doctor,I have decided to terminate my employment with the company as of 2023-11-05.This decision was not taken lightly,but after careful consideration,I believe it is in my best interests as well as the company's interests.
